Registration For Returning Participants

Parents are required to provide their own coverage as outlined in the waiver that each parent is required to sign in order to participate. The coach should stay with the injured athlete to supervise and attend to their needs. No child will be allowed to participate without a signed waiver form. Each participant must have a waiver form signed by a parent or guardian. The injured athlete is the most important person needing supervision. Physical examination, as stated in the instructions on said form, is mandatory before any participant will be permitted to play in a game.
